Plympton fire department cited for failing to explain how rescue workers determined Hingham girl dead


State: How did they know Torri Wightman was dead?

BOSTON —

The state says it can’t determine whether Plympton’s fire chief and EMTs acted appropriately at the scene of a 2006 accident that took the life of a 16-year-old Hingham girl because a mandatory report was not filed.

Victoria Wightman, a student at the South Shore Charter Public School in Norwell, was one of three passengers in a car driven by another teen. Her mother, Lucy Wightman of Hull, filed a complaint this past October saying no one tried to resuscitate her daughter at the crash scene in Plympton because the local fire chief had determined she was dead.

But the fire chief, David L. Rich, told state investigators said he was told at the scene that one person was dead, and that he didn’t remember assessing Victoria Wightman’s condition himself.

The Office of Emergency Medical Service, the state agency that oversees emergency medical services, now has cited the Plympton fire department for failing to explain how rescue workers determined that Victoria Wightman was dead and beyond help at the scene of a two-car crash on Route 106.

The agency has put the department on notice of a “serious deficiency” because the department didn’t file a complete trip record for the May 16, 2006 accident, in which Wightman was covered by a sheet as she lay in the wrecked car.

While accident incident reports were filed by Plympton EMTs, state medical regulations require that a trip report be filed for every accident victim, regardless of their condition. Trip reports include the initial medical assessment of victims, as well as details about the care that’s given en route to the hospital where they’re taken. A copy of the report is to be left at the hospital.

According to a state report issued last week, Rich told state investigators that when he arrived at the scene, an off-duty Plympton EMT told him that there was one dead person and three seriously injured people in the car.

Didn’t remember

He told investigators that he didn’t remember doing his own assessment of her condition or covering her with a sheet.

State officials said they can’t determine whether Rich and the EMTs acted appropriately by not taking Wightman to the hospital, because there’s no trip report to review.

“There’s no record of what they saw,” said Renee Lake, the state office’s compliance coordinator. “That’s the whole point of this (report). We are unable to determine what she (Wightman) looked like, or anything.”

“Just because a patient is not breathing, it doesn’t mean that they shouldn’t be transported to the hospital,” Lake added.

Rich had no comment about the report on Monday. He and his department must now file a “plan of correction” by Feb. 13, telling the state agency what they’ll do to avoid another such failure. The Plympton department was also cited for not being properly registered to carry and use controlled drugs or medical equipment such as needles and IVs on its ambulance.

If the department fails to respond or doesn’t file a proper plan, it could eventually face $500-a-day fines until it is in compliance.

Mother complained

Victoria Wightman was one of three passengers in an Oldsmobile Cutlass driven by Erin Leith, 18, of Kingston. Wightman was fatally injured when Leith lost control of the car as she went through a large puddle and swerved into the path of a minivan driven by Mary Hall of Halifax. Neither Hall nor her three children in the minivan were seriously injured in the collision.

Leith and the other two girls in the car were cut free and taken to hospitals with serious injuries. While emergency crews worked to free them, Wightman lay covered with a sheet inside the wreckage of the car.

In her complaint to the state, Lucy Wightman also contended the fire department failed to call the closest ambulance to the accident for help.

According to the state report, Plympton’s town ambulance was dispatched at 11:02 a.m. and arrived at the accident at 11:09 a.m. An ambulance from the American Medical Response medical service was dispatched from the fire department at 11:09 a.m. and arrived at 11:15.

Leith had been scheduled to go on trial today on charges of speeding and misdemeanor motor vehicle homicide. That case has been continued to April 29.

Lucy Wightman said she hasn’t yet decided whether to file a lawsuit against Rich and the fire department.

This makes me so unspeakably angry. Such callous, negliigent behavior. It's unbelievable to me to think that people's lives are in the hands of people who don't double/triple check on victims....and then to "not remember" what they did?? Lucy, this only compounds your grief.....I am so sorry.......

Were you paralyzed there in that tin box unable to move, unable to show any life? Did you give up once they gave up on you? Once you heard no one would come to help did you die so quietly alone? Did you want someone to hold you, to breathe for you, to help you? Did the sounds call you but nothing responded? Were you so afraid no one would help you? Couldn’t you hear the last pulses so deep and loud in your ears while they worked around you? Did you cry inside wondering why no one came? Did you finally lay down inside and try not to hope?

I am not dying this way as you. I feel some of these things.

Torri was a fighter

i don't know if she was able to think and if she could it is so hard to think about what her thoughts were

they should have checked on her

they should have helped her

i am irate.. this destroys me

i am sorry Lucy

i am sorry that you have lost your daughter

that you are here living without her destroyed and having so many unanswered questions

so much more i would like to say but my words are not coming out as i would like

how do i possibly express all that i feel

i can't

just hope that you can sense some of it

to 'no one' i am glad that you feel

i am also glad that you are not dying

but not sure that you can say as 'you'.. Torri did not have the choice as you do...

Oh Lucy I am so sorry. I think that you always had a feeling that some thing about the way this was handled was so terribly wrong. Your instincts were right. Shame on these people. It is beyond criminal. She would want you to make sure that nothing like this is ever allowed to happen again. I know you are tired of fighting. I know you are tired of being strong. I wish I had more to offer than prayer.

NO trip report;or destroyed along the way so there is no written evidence of negligence???

No trip report AND "can't remember"; excuse me, but one has to wonder how many "dead bodies" the Plympton Fire Department encounters on an annual basis. I could be way off, but I'm thinking maybe not that many. How can one not remember the cirmcumstances surrounding such a scene? Who the hell put the sheet over this beautiful child?

You need to find out who filed the death certicate. This needs to be done in all cases. In it there will be cause of death with details. Tough stuff if you want to find the truth.
